FIDELITY ADULT CARE LLC intends to provide residential care and support services for eligible MaineCare members under a Home and Community-Based Waiver for Adults with Intellectual Disabilities/Autism who are 21 or older, meet institutional level of care, and who choose to live in the community with the support of this waiver.
This Home and Community-Based Services waiver is designed to maximize the opportunity for members to achieve the greatest degree of self-sufficiency and independence chosen by the member. Member choice in all services and components of services is a primary goal of this waiver.
The principles of conflict-free care coordination, services provided in the least restrictive modality, and the effective use of assistive technology for communication, environmental control, and safety are inherent to this waiver.
